Job Description

Looking for a change of scenery and a chance to help others. Provide skilled home health physical therapy services to patients across the Arlington, WA service area. This travel Physical Therapist will evaluate functional status, develop and implement individualized plans of care, monitor and document patient progress in accordance with state and federal regulations, and collaborate with the interdisciplinary team to achieve patient-centered goals.

Minimum Requirements

  • Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program.
  • Current Physical Therapy license in Washington State, or a compact license that grants privilege to practice in Washington.
  • Current CPR (BLS) certification from AHA or ARC.
  • Valid driver’s license, vehicle insurance, and access to a dependable vehicle for patient visits.
  • Ability to meet productivity expectations (30 points) and complete documentation according to agency policy and CMS requirements.
  • Experience providing home health services or visiting patients across a service area. Coverage area includes North Snohomish County, including North Marysville, Granite Falls, Arlington, Camano Island, and Stanwood.
  • Bachelor’s degree minimum education requirement.

Preferred Qualifications

  • Home health or home care electronic documentation experience and familiarity with Home Base strongly preferred.
  • Experience supervising PTAs and home health aides when permitted by state regulation.
  • Prior experience meeting agency productivity standards and completing timely progress notes and 60-day summaries.

Assignment Details

  • Shift hours: 0800–1600 (M–F schedule noted as M-F 8am-4:30pm in order notes).
  • Schedule expectations: 8 hours per day / 40 hours per week.
  • On-call/weekend requirement: candidate should have schedule flexibility and ability to participate in a weekend rotation and potential observed holiday availability as needed.
  • Mileage reimbursement provided between visits (from first patient to last patient).
  • Coverage area: travel to patient homes across the service area; candidate may be expected to visit patients throughout North Snohomish County and nearby agency service areas.

Impacting Patient Care

Provide in-home evaluations, create and implement individualized PT plans of care, and report patient response to treatment to the interdisciplinary team. Deliver evidence-based interventions to relieve pain and restore or maintain maximum function. Complete timely documentation including evaluations, progress notes per visit, and discharge/summary reports every 60 days. Participate in staff conferences, in-service education, and maintain continuing education to comply with state licensure requirements.
